The definition of “I’m alive” according to the Federal Government.
As soon as your driver’s license is changed, you should apply for a new social security card with your new name.
You’ll need a copy of your marriage certificate, your driver’s license (or something reflecting your new name) as well as another form of ID (birth certificate, passport – something that proves your identity). At least one form of identity should have your photo.
You’ll need original documents – they won’t take copies or even notarized copies. You can either mail the form with the appropriate original documents to your local social security office (found at https://s044a90.ssa.gov/apps6z/FOLO/fo001.jsp), or you can visit the office in person.
The most up-to-the-minute form is at:

For more information and detailed instructions, go to www.ssa.gov/ssnumber
You can mail in the form with original documents, or you can go to the Social Security Office in person. If you go in person, you MUST go to the Social Security Office in the county of your permanent address.
NOTE: Mess-ups are possible! When I got my card, they made my new name Nina Diane West, when I had specifically asked to have it be Nina Sharma West. You’ll have to sign a document asserting that you have asked to have your name changed, to be sure to CHECK what they have as you new name. And, be patient. The Social Security office is about as functional as your run-of-the-mill DMV.
